[Hi- just got here, first time caller, long time guest...anyone wanna help me out? I'm trying to makey friends-like but this question may get some vet's upset (yikes-please forgive) I'm used to a dial up. I find it to be safer and I am fairly patient. Currently I have an issue with a friend who's apparently uninstalled (and emptied rec. bin!) his Enternet 300. He uses his laptop for work purposes (IOW, issued by his job), so I'm in a pinch here with some limitations on what I can download, or more specifically, whatever I download or buy has to work. I researched on Enternet 300 software, seems it's unstable or whatever with that particular OS , and all in all has more bugs a rotting banana-flavored-monkey, but whatever, I just need to know what to do. Does he need this software or is there something better that is more stable? I can't download AIDA in good conscience so I got the following info DXDiag...One more thing-he has a dial-up also, which KINDA works, but IE isn't takin' to it very well. He's using extremely crappy AT&T (THIS COMPANY IS A JOKE) DSL--> laptop-->OS Win 2000 5.0 service pack 4---->Processor Intel x86--> 1800 MHz--->Memory 254 MB RAM--->page file 164MB used , 707 MB available--->DirectX Version: Direct X 7.0 (4.07.00.07.00)--->DxDiag Version 5.00.2134.0001--->
DX SETUP PARAMETERS: NOT FOUND Really I know it's a simple thing with hopefully simple software that'll integrate smoothly. BTW- he does NOT have to use Enternet 300 because ATT is the only one who uses it and his company let him choose any which provider he wanted. Called ATT and they say it's his company's software, his company says it's theirs...Please, if it's too technical pleasee add in the jack-ass version-my brain is non-functional when it comes to acronyms ....sorta. I recommend downloading and running Reimage. It's a computer repair tool that has been proven to identify and fix many Windows problems with a high level of success.

Ok first with windows 2000 you need a ppoe shim (enet 300) I worked for a dsl company and we also used a either tango manager (crappy) or enet 300 for our dsl customers. I have had much less problems with enet 300. ATT isn't the only group to use enet 300 or if using windows xp the built in pppoe connection. For a few days now, whenever I've clicked to go to a different webpage in IE11, most of the IE screen has gone black until it gets to the page I want.
Then this morning, when I powered up I got the above message.

I searched here and found many threads, most of which had common advice to run sfc /scannow. I ran this and now have the CBS log on notepad. It shows several programs which couldn't be repaired.
I haven't got MSPaint and it look too long to snip. It's 21 pages long on Word and I'm not sure if that's the best way to send it to you. I'll try - and watch what happens.

In case it's relevant, I still haven't been able to install the latest Microsoft updates - I've tried every day but just can't get through.

Edit: Some of the threads were in Games, others in programs and some in Drivers. If I've put this in the wrong section, would some kind person please move it for me please.
Edit 2: again, in case it's relevant, I just remembered that when I opened IE11, the Norton task bar was showing although it was ticked at the top. So I restarted IE11

When i try to open this program called Novelty it says "Failed to create Direct3D device"
and "No image handler for type 17 defined".
And then it opens a note and the one written says:

Answer:Failed to create Direct3D device

It sounds like the onboard graphics is insufficient to run the application. You could try installing the latest graphics driver available but it might not help.

every time i try to run a program that uses direct 3d, or any 3d for that matter, it crashes out or errors out. i ran DXDiag, and all tests check out, except direct3d, i get the fallowing messages:

its a Geforce4 4400 128mb, it just all the sudden started mid a game, then i couldnt boot the game back up. please, ,someone help me out here.

Hi,

I have a NVIDIA GeForce 8600 GT. Now I was trying to run a trial version of Badaboomit (video converter) on my pc and it came up with an error and made an error log. In the log, it said: "GPU 0: NVIDIA GeForce 8600 GT
FATAL:ERROR: failed to create D3D device (0x8876086C)
GeForce 8600 GT GPU selected

FATAL:"

I thought maybe it was a problem with drivers so I downloaded the latest and installed it: No Cookie.

Then I proceeded to uninstall the drivers, and clean it using an "driver sweeper". I installed the new drivers again... still, No Cookie.

Before writing off the application, I decided to check some other things...

Nvidia Sterioscopic 3D setup wizard:
Error: Failed to create D3D Device.

PhysX_Screen_Saver (from NVidia site):
Error: Unable to create device
Error: Unable to setup 3D device

So as you can see, there is a pattern here.

Badaboomit is a video converter that uses CUDA to do all its processing on the GPU. I really wan't it to work and as you can see, the problem is not limited to Badaboomit only.

My PC:
Windows 7 - OS,
3GB - RAM,
Intel P4 HT 3.2GHz - CPU
SATA - HDD
NVIDIA GeForce 8600 GT - GFX Card

guess what...

about 6 months ago, I stopped working with DirectX... Uni finished .

I forgot that I had directx runtime and debugging tools and the tool was set to use a debug version of directx. I told it to use the retail version and vioalla! everything works!

Hope this helps some unsuspecting graphic developer... I feel like an idiot now.

Recently when I tried to open Device Manager on Vista, I got an error 'Failed to Create Empty Document' Device Manager never opened.

Anyone know why it is doing this and how to fix it?

For me, it was a problem with user32.dll.mui, so I had to change the one i was using for a backup. So ye, problem fixed now.

I received this error after trying to play europa universalis 4, after updating with the latest windows 8.1 updates. Yesterday all games worked fine, today, none of them run at all.

I've been searching for a solution but all i can find is update your graphics card / DirectX / install the Dx in the game folder itself, all of which i did, but none of it worked.


I've tried pretty much everything i could find about this problem, anyone have any ideas? Only difference is i am running all of this off a laptop with an intergrated graphics card though, and for the next few months my laptop is all i have in terms of a pc. Graphics card is Intel HD graphics 4600, which worked just fine a day ago.

You are certain you have only HD 4600 graphics and not also a discrete Nvidia graphics chip? Does device manager show only HD 4600 graphics adapter?

Does anybody know what this could possibly mean? Whenever I stick a DVD in I get an error message that says "Creat Overlay failed. Lower your color depth or resolutiojn and try again." I did what it said first I shrunk my resolution down to 800x600(an event wich will permanently scar me), that didn't work then I reduced it to 256 and got the error again. I recently had uninstalled the DVD software(Intervideo WinDVD) because my computer was very screwed up so I basically uninstalled everything I didn't use everyday(it didn't fix it). I was able to watch DVDs fine with the same default settings, I have tried multiple DVDs and it still happens, nothing has changed on my system, I didn't delete any shared files. What could possibly be wrong?

Question: Enternet error

While teaching a student a musical program on her computer I received this error: "Enternet has caused an error in KRNL386.exe." and the computer froze. She told me that her computer does this about once per day. She's running WinME on a Dell computer with 128mb RAM. I didn't have too much time to check anything else. I've never seen an Enternet error before. Does anyone know what this is and why it's happening? Thanks

Hi again!I'm running XP Home, v 2002, Service pack 2.I just upgraded from windvd 4 to windvd 7. (powerDVD) It looks impressive but whenever I try to play a dvd I get the message -"Create Overlay Failed - please lower your screeen resolution and color depth and try again" I tried that but no luck. I was later told that Messenger could be causing the problem, but I don't have messenger installed.It's driving me nuts, and I can't even watch a nice movie to calm my nerves -   One more thing, I recently also installed "AnyDVD" - could that make a difference, friends say it shouldn't?I tried running AnyDVD in Windows 98 / me compatibility mode but again, no luck.All help / advice is very welcome, thanks in advance.

Well guy try this one:right click on Desktop and select properties then select Setting Tab and press Advanced button. A dialog will appear, select the Troubleshoot tab. Here lower the Hardware acceleration and then try to run DVD again.DW

I have a new laptop running Win7 Pro 64 bit. I did an original system image when I first got it, so I'd have a good image if I had to revert to it.

I installed Norton and Malwarebytes, applied Windows Updates until the machine was caught up, installed my Office apps, Quicken, and Printer software, and applied all the updates I could find for the additional programs, and then made another backup.

When I made the second backup, I also tried to create a system repair disk. It failed, with an error code of 0x80070057. I have attached a screen shot showing the error.

I found some old threads on this board with various suggestions of how to approach this, but I have also experienced the fallout of trying a solution suggested for someone else when the actual underlying issues were different. So I figured I would start here.

Recommendations, anyone?

I'm having difficulty access media files from my simpletech external hard drive. It pops up immediately upon opening. However it freezes as soon as I click an icon. I try to make it respond and it takes a couple minutes before the folder opens. When it does, and I want to create a new folder, after some time in deep freeze, it gives me the following message:

Unable to create the folder 'New Folder'. the request could not be performed because of an I/O device error.

The media files in the folder open up with no trouble, just your typical pc trouble.

The funny thing is, when I'm in the main folder (which freezes on its way back up the folder heirarchy), I can create a new folder.

I am nervous for the fate of the external harddrive. This is where I back up EVERYTHING.

you may think this is an odd idea but it will save you from lots of small culprits just backup the things that you can on your system and well connect your ex hdd and full formate it (not quick) . Then disconnect and reconnect it and try by copying files if the problem comes once again or not. If problems not come then before copying back your old data check them with you antivirus with latest virus definitions. u may think what is relation with i/0 error with viruses but it is i have suffered this in my 4 gb pen drive. After all this replay us quickly.

So when i tried to install the os, i got an error lik this "failed to create partition on selected unused space. Error: 0*80042453". I have no idea how to proceed further. I read some posts of the guys having the same problem but they did't help much.

Hi...LOW LEVEL FORMAT TOOL...use it to low level format your drive...then scan it with HDTune...Then try to create a single partition with low formating method. Then run CHKDSK /f /r on that partition. Read S.M.A.R.T. status... if bad...replace disk.

Hey guys,

I have a Windows Vista Home Premium 32 bit and I get this error after it has been sitting idle. It does not go to standby but screen shuts off. Only way to return computer to original state is to hard boot it and restart it.

Anyone have any advice?

Ok, I've looked everywhere and tried tons of solutions to fix this problem. I've uninstalled the drivers for the USB HDD too many times to count and re-installed them, I've uninstalled all the USB drivers and restarted the comp, and more! Here's the problem:

I have a removable HDD for an Omnifi DMP1 car stereo. The HDD has a USB plug and a power plug, which I use both (duh). I plug in my HDD to my computer (Windows XP SP2 Media Edition) and the New Hardware Wizard runs in the bottom corner. Then it says there was a problem installing the hardware, and then the drive can't be found. So I try the same thing on my laptop (XP SP2 Professional) and it runs New Hardware Wizard in te bottom corner. However, it says the installation was successful, and it can see my drive!

What the heck, I'm totally stumped on this? If you could help at all, that would be great! I am leaving on a trip tomorrow, so I won't be able to do anything until I come home on Sunday.

Let's talk only about the computer on which the external USB drive doesn't work.

What are the system specs of that computer (see the link in my sig about posting system specs). Don't forget the informations about your Power Supply Unit (open your case and read what's written on the sticker that's on the side of the PSU block). If it's a computer you bought as a whole package from some known brand then brand and model will do but the infos on the PSU will still help.

Are you 100% sure the USB ports on that computer are USB2.0 ports ?

Hi,

I accidently deleted a graphics card driver but managed to get it back, but before i deleted it i used to play the game pkr (3d poker) but now when i load it, it comes up with an error saying "Could not initialize client - Can't create a D3D device - -2005530516, -".

Also all my folders on my desktop don't appear. The only way i can get this up on screen is by pressing the keys ctrl + alt and delete and then typing in desktop.

All this started to happen when i tried to connect my laptop to the television.
I am using Windows XP media center. It's a a Toshiba Satellite M115-s3094.

Can anyone please help as it's driving me mad as i am very close to smashing the laptop in frustration.

I don't know the driver its using as all it says is unknown. If anyone can help or even give me an idea on how to fix this then i would be very grateful.

Sounds a little bit like the Direct3D were not enabled in you graphic settings.

Please start the DirectX using the command ?dxdiag? and enable all 3D settings.

Furthermore it would be advisable to remove the installed 3D poker game and then to reinstall it again. This should help to reconfigure the game graphic settings which are necessary to run the game properly.

I have two WD "My Book" 3.0 USB drives.  Previously, they both worked fine.  Now one of the two drives is getting a device descriptor request failed message.  The other drive is fine.  I have tried different 3.0 inputs on the system both in back and on top in the front.  Any ideas on why the same drive isn't working?  I have tried both on other systems and they work fine.  The other systems I have tried them on are WIN7 and WIN 8.1.

Hi there slovey  Have you, by any chance tried with another USB cable? I wouldn't make sense to change things as the device works on other systems but it wouldn't hurt to use the other My Book's cable with the problematic one just to rule this out as a potential cause. I would also check this thread and see if the offered solutions there help.  Keeping your OS, BIOS and drivers up to date is also an important part of the troubleshooting process. I'd also check if there's a new update specifically for the driver responsible for the communication between the drive and the system. Post back if you need more info! Captain_WD.

Hello, this morning my keyboard stopped working and the device tab says the line in the title with a yellow triangle on it as well. Everytime i try uninstalling them and reinstalling the same thing pops up. All usb ports are up to date. I have desktop and the problem is only with my keyboard (corsair k95). all usb ports work and all other devices work in every port. The solutions in the youtube videos and forums don't work for me. I really want to fix this as my keyboard is only 2 months old. I am desperate to solve this please help me¨find what the problem is so I know if I can fix it or not.

When I wake my laptop from sleep mode, I get error msg, "The selected boot device failed. Press <Enter> to Continue." When I do, the laptop shuts down and I have to do a reboot using the power button. This is a new laptop, came with Windows 10. There is no removable battery and I do not have a cd drive nor a USB plugged in while it is sleeping.

I have a Toshiba Satellite U205-S5002. I have Windows XP. I try to download drivers but I can't. I have no disc. The (TouchEd Error, Retrieval of "Thotkey" Failed. Error code 0x00031402, 0x00000002) this error ups when turn on the laptop. Also the Ccommon error. I have no sound at all. Any help I will appreciate.

Hi guys,
I am trying to get my microhpone to work, and it doesnt anymore, it's definately not the mic as i have tested it on another comp, but it won't work on this one.

I'm running a Soundblaster Live! 24-bit sound card on windows XP SP2 , and the Creative Game Port has a Yellow- (!) next to it with an error code 10.

Anyone know how to fix it? Or why it does this....

I tried re-installing drivers, changing the sound card to a different PCI slot, and disable the on-board audio, on my Asus A7V600.

Here is what I found on the code, but from what I have read, it doesn't sound as if it will help any:

Code 10

If the device has a "FailReasonString" value in its hardware key, that string is displayed as the error message. The driver or enumerator places this registry string value there. If there is no "FailReasonString" in the hardware key, the following generic error message is displayed:
This device is either not present, not working properly, or does not have all the drivers installed. (Code 10)

Try upgrading the device drivers for this device.
Solution button: Update Driver

To resolve this error code, make sure the device is connected to the computer correctly. For example, make sure all cables are plugged in fully and that all adapter cards are properly seated. Follow the suggested solution button and update the device driver. It may be possible to remove the device and redetect it using the Add New Hardware wizard.

having trouble installing an Epson WF-2530 printer onto win 8.1 machines.
i have 3 win 8.1/64 bit machines. it will install and is working on only one.
used the same installation disc, and install process, and cables with all 3 machines.
googling the problem shows plenty of stuff mainly updating drivers. done that, still no good.
Not sure why it works on only one, a generic clevo laptop w550eu.
thoughts???
the installation process stumbles when it asks you to connect the printer cable to the computer.

Was trying to plug my Toshiba 1tb External hard drive in to retrieve music off it and it wont recognize the drive. Device manager revealed the yellow triangle and error message. It works on my wifes hp streambook and also my kids stream book both are running wondows 10 like mine. I have tried nearly 10 different suggestions from google searches and have gotten nowhere. The only thing I couldnt try was turning off the usb in the power options, for some reason mine doesnt display that option. My notebook only has 1 usb port so I cant try a different port. Any suggestions or fixes?

Hi, I needed help in solving this problem. I was playing Heroes of Newerth when suddenly I lost the game. When I reconnected, this error d3d_create: devicecreate() failed fatal error pops up. I tried checked using AVG PC tuneup about this error and it says that the driver was incorrect and that i have to update my driver. My graphics card is NVIDIA Geforce 8500 GT. I tried getting the latest version of this online at nvidia.com. When I installed it and restarted the pc, it still doesnt work. Everything works fine before but it just happened recently. I also tried uninstalling and reinstalling this but the same error pops-up. It's not only on this game. Any other game which I believe is "heavy" would not work. I appreciate if this would be resolved without me having to buy a new one. Thanks. :wave:

T-540p - Windows 10 Pro x64 (upgraded from Win 7 Pro) I'm having the Inaccessible Boot Device problem discussed elsewhere here, and I need is to have a working bootable DVD or flash drive. Couple of questions. I created a system repair disk on my hub's T-530, but mine wouldn't boot to it (non-system drive error) when I changed the boot order to boot to it. Is it still possible to create a disk or flash drive to actually boot to Windows vs simply allow it to be reinstalled? I have a fully functioning command prompt, but sfc does turn up some anomolies, and I assume I need to be booted elsewhere to fix them? I see all my files on C and a backup drive just fine at the cmd prompt, and was able to get all  my files to the backup drive OK using xcopy (restoring system without losing files option does not work).  Chkdsk showed a few bad sectors, plus the "MFT Bitmap attribute is incorrect"--is that indicative of an impending disk fail? Given safe mode is not even showing as an option, I'm guessing the boot manager etc has been fubar'd. Is there any way to repair this? Will reinstalling Windows 10 even affect that? If the HD is fubar'd I'm guessing not. I'm assuming I'll need to work with Lenovo on this as it's still under warranty, but hoping there's an easier way. TIAelaine

Blank media or no media is present in optical drive. Test requires media with digital data (Resource CD, Installation CD). Where do you get these if they did not come separately with computer when purchased?

By a Resource CD, it may mean any digital CD. Did you get any CDs with the PC when purchased? or do you have a cd that you have written on (burned) and that you know you could read from? Do you have cover CDs from computer mags? Anything that the drive should read.

Umm, I had this error when i tried to open a file =)
All i did, was move the files somewhere different, the error was that it was within too many folders.
The destination for the MP3 was far too big for my PC to comprehend.
So, that's my take on it =P

If you get it at random intervals, then maybe it's some game/application or system files that've been installed Beneath loads of folder, thus
the destination folder would be too long and yeah...

Hi,

Get no sound when I try to play an MP3, or watch a news clip. Re-booted, unplugged audio/speaker cord, to no avail. See images below.

I don't know what caused the "multiple NIVIDA drivers". Updates?? The Multi-drivers may be the problem, I'm only guessing. No idea what the "unknown driver" is either.

Did I supply enough info/images or is more needed? Please advise.

I upgraded to Windows 10 a while back. Every time I open the computer I get a pop-up notification telling me the USB device is unrecognized. I have no USB devices in any of my 3 USB ports.
I sent my computer in twice. No help. They just uninstalled everything and reinstalled everything and told me it should be fixed. It is not. I then went about updating every driver in my Device Manager. I uninstalled the Unknown USB Device and then updated
other drivers. Certain drivers (such as PCI Express Root Port #3 - 9C14) when updating would also reinstall the Unknown USB Device, which I would then uninstall and keep working down the list. At the end of updating EVERYTHING I scanned for hardware changes
and "found" the Unknown USB Device. 
From what I can tell, and I know very little about this stuff, my computer can't name one of my USB ports. Since it can't figure out what it's called it can't download the proper driver for it. Since it can't download the proper driver it can't work properly.
The practical result from this is that one of my three USB ports does nothing when I put in any USB item (connect my android phone, thumb drive, presentation clicker, etc).
I've looked on tons of forums. My current thought is that if I can tell my computer what the deice is or download the right driver and tell my computer to use that driver for this device it should solve my issue. But I don't know how to know what the name

I have dell latitude E7470 with windows 10. I have problem when I want to install dell dw5811e driver. I download latest software driver from dell support, run it and wait them to finish. Afterward software driver is configuring mobile broadband device, it always freezes at 37% with message "Fail error 82". I try with older version of software driver. It is still the same. Could anyone suggest how to resolve the problem.

Hi, guys.

When I want to install certain programs, there is a set of error messages that makes the installation fail. They're as follows:

"c:\PROGRA~1\Symantec\S32EVNT1.DLL. An installable Virtual Device Driver failed Dll initialization. Choose 'Close' to terminate the application."

If I push "ignore" then this message appears:

"c:\Windows\system32\autoexec.nt. The system file is not suitable for running ms-dos and microsoft windows applications. Choose 'close' to terminate the application."

I have a feeling that this is caused by a Norton antivirus program I installed couple years ago that I had a heck of a time removing. Could there be a chance that some parts of the program is still lingering to cause this?

I've been having this problem for the longest times and threw away a bunch of software because I simply couldn't install them. It's soooo frustrating. Please help!

I'm attempting to help my daughter fix her USB issue, so the information I have about the computer is minimal at this point. The computer is and HP Pavilion a6710t Desktop PC. It has a Foxconn motherboard. Here's the story, She's been using this computer for a couple of years, was running Window Vista. One day her wireless mouse and keyboard quit working. I gave her a usb mouse to try to no avail. The message when we plug any USB device that previously worked on her computer is 'Unknown sub device, device descriptor request failed'. After doing much research online and attempting a multitude of fixes from online sources I had the bright idea (being the computer genius that I am (NOT!)) to update her Operating system to either Windows 7 or Windows 8. She chose Windows 8. Here I'm thinking if I wipe the hard drive, do a clean install of Windows 8, the problem would be fixed. Boy was I wrong, still no USB ports are working. Makes no sense to me unless it's the hardware itself. Any suggestions? Ideas?
 

Answer:USB ports inoperable, error says, device descriptor request failed.

Based on what you have tried, it does seem to be a hardware error. One more thing to try is check the CMOS battery's voltage. It's usually a CR2032 rated at 3 Volt. Carefully remove it after unplugging the PC from the socket outlet, and check the voltage with a digital multimeter.

I was in the middle of backing up my failing hard drive to a brand spankin' new internal drive when my computer completely froze on me. I restarted and received this error:

I've already gone into the bios and made sure it was booting on the right drive, and it was. I've switched to every possible boot device and they all give me the same error message. I've also ran the windows installation disc and performed a startup repair multiple times to no avail. I've even checked for loose cables but everything seems fine, and even if my old hard drive has finally failed, it still won't boot on the new one either.
 
I've also ran 'bcdedit' in the command prompt and it showed that it was booting on the correct drive. so I'm really just at a loss here.
I've been having non-stop computer problems for about 2 weeks and I really just want it to stop. (not to mention now I'm really concerned I've lost all my files for good)
 
Anyone have any suggestions?
 
(by the way, I'm really not the most tech savvy person, so try explaining your solutions like you're talking to a small child)

Hi,
I booted up my computer today to find that whenever I try to select an option from the booting screen, the whole computer froze, as in, the timer counting down the auto choosing feature froze, I couldn't move anything. I thought at first it was the keyboard, as I use a razer product, I thought the drivers weren't loaded and it froze it. I tried it with the standard keyboard it came with and still froze at the screen (by the way, its option chosen is startup repair, im trying to get to start windows normally). I thought it was the monitor next and switch it around to no avail. I then dug around for a few hours to find my window repair disks. I get a prompt on screen to press a button to continue, which then freezes my PC. If I leave it at that screen for a while, startup repair is launched and I get a Status: 0xc000000e and the info says: The boot selection failed because a required device is inaccessible. Also, choosing any option with my motherboard BIOS also freezes/crashes the computer.
I really need help with this guys, thanks.

Can you boot in to safe mode or a clean boot state? Difference between Safe Mode and Clean Boot State in Windows
Then try and see if you can run the system file checker. System File Checker: Run sfc scannow, Interpret sfc.exe logs in Windows 7
Else Run Startup Repair from the System Recovery Options. Repair Boot Problems with Windows 7 Startup Repair
Else Rebuild BCD using bootrec.exe. Error message when you start Windows 7: "The Windows Boot Configuration Data file is missing required information"

Okay, so this just happened out of nowhere. Nothing seems to have led up to this -- the system booted and shut down successfully, and no software updates or installs were done during the period of time it was on when it was last working. I've attempted many things, but none seem to work. /ScanOs reports no OS detected, while startup repair said the OS was booted successfully. bcdedit also shows no directories linked to "unknown". CHKDSK reports no bad sectors, so such can be discarded. This happens every time I attempt to boot to Windows 7 (Home Edition, x64). I have a copy of Fedora 17 set to dualboot with it, but it hadn't been booted for over a week before the event. Any suggestions?

Built this pc, thought everything was running stable but i got another BSOD earlier. After it has rebooted i get a message saying 'the boot selection failed because a required device is inaccessible 0xc000000e'. After rebooting once the pc will boot fine to windows 7 ultimate 64bit

Currently i have the two OCZ Agility 3 (with latest firmware) running on the Jmicron SATA 2 ports in RAID 0 on my Sabertooth X58 motherbood and i randomly get this problem. Doesn't seem to be any pattern to it. HDTune shows no errors on the Hard drive error check.

The odd thing is that this didn't happen when it was plugged into the marvel SATA 3 controller, it was just slower... because marvel are terrible > so i have figured out that it must be something to do with the RAID controller on the motherboard? I have the latest JMicron SATA drivers from the official ASUS website.

Another curious thing (not sure if it is related but i am going to post it anyway) is that before i rebooted it earlier HDtune was benching the hard drives and showing that i was getting 2.5gb a second which is impossible.. don't know if this has any relevance however..

I did a memtest earlier in the day to make sure that the RAM was running stable. It did one pass and no errors were found so i am ruling out the ram.

I use Windows XP. I installed and ran the Avast grime fighter and then found two of my programs - one for tax preparation and one for backing up to an external hard drive - won't "open". Instead, I get the error message "failed to create empty document". I uninstalled the programs then reinstalled them, but no change - I still get the message. I saw, by googling the message, that it may have something to do with a windows registry file, but I really don't understand what that is or how to get to it or how to fix it. I contacted the Avast folks and they said they could put me in touch with their microsoft folks for $180! I am at a loss as to what I should do. Please can anyone help me figure out what to do to get these programs to run? I really need the tax program especially. Thank you.

Hi,
I am new a member and was looking for help with the next steps of how to resolve the following error message 0x8000ffff "Failed to Create Volume Snapshot". I got this whilst trying to clone one disk to another (which have the correct amount of space to make a clone) so far on all other machines i have not got a clone error until now.

Followed the advice on Macrium's Knowledge base:
VSS Error:0x8000ffff - Backup aborted! - Failed To Create Volume Snapshot. (VSS)

All Solutions have been tried but the same error occurs. The service is set to automatic but according to the user it has not started.

However, on my own laptop Macruim clone goes through without my Volume shadow copy being set to automatic.

Anyone got any thoughts how i can get this clone to go through?

UPDATE -

I got an some more information from my persistant googling to find another piece of the puzzle.

On the affected machine i did the following:
Opened a command prompt (with elevated privileges) and type ?vssadmin list writers? (without quotes).

This command listed all of the VSS writer i had installed on the machine. This came back with no errors.

I then did a search in Event viewer around the time i did my clone last week. I did a find and searched for 'VSS'. This came back with many errors about the following service:

Oracle VSS Writer

Hi,

I just started receiving this error:
UA StartSignalReceiverFromOuterThreadError: Receive Socket Create Failed!"

With the exception of Curse Client I can't connect to the internet. No browser will connect(firefox, IE, Chrome, Opera). Steam won't connect. Email client won't connect.

Other computers at home can connect. Not problems with router.

Event logs show Critical errors with
NVNetworkService
PlsvcV2
Sendori
Store-Licensing
Application (This is NVidia)

I can add more info from the logs if that would help.

The only thing I've done is a System Restore. It didn't help.
I had installed Assassins Creed which installed DirectX.

I've searched the webs and forums and can't find anyone with this issue but 1 guy 8 months ago, and there was no solution posted.

Any help would be greatly appreciated. I have soooo much work to get done and I'm freaking.

One of my users is having this issue. She is is using Windows 7 Pro x64 bit with Internet Explorer 11. Everytime she tries to access a specific website IE stops working and gives the following error message:
create file mapping failed (6), wc_registry_mmap_name
No one else is having this issue when visiting the same website on their computers, it only happens to her. I reset IE's settings on her computer but that didn't fix the issue. I can use a different web browser on her computer to visit the website, so I
know the website isn't the problem.

whenever i run hardware scan, i get error on storage device, mostly on SMART short Test and SMART drive self test.after error result i scan it again and then result is Passed.i just recieved notification of 'critical errors occured on hardware u need to scan it'. and i run the scan but  hardware scan freezes(it happenes mostly whenever i myself run a scan without getting any notification to run a scan) and then it gave me this failure result(refer to attached image below).Reply as soon as possible.Please help me to resolve this issue, i am too much worried about it.will be very greatful

I'm having a lot of trouble with this western digital hard drive, originally it was for a sky box I took it out and I want to use it for my PC, I've tried formatting the disk via command prompt but I'm having no luck.

Got this when trying to create a new volume/partition

In partition wizard, I'm getting "bad disk"

I was just working happily when my Microsoft Bluetooth Mouse just suddenly stopped working.
The Bluetooth settings page says "Bluetooth is turned off" and there are no options on the settings page or the system tray to turn Bluetooth on.
The Device Manager now has "Unknown USB device - device descriptor request failed" where it did not before.
Other devices all seem to be working fine - USB devices plugged in are being recognized and work properly. Only BT seems to be broken.
I've tried:
uninstalling the device and rebooting
installing the latest updated chipset drivers from the dell website
windows updates
pretty much all of the advice here: http ://troubleshooter.xyz/wiki/fix-usb-device-not-recognized-device-descriptor-request-failed

Any other ideas?

Answer:XPS 15 9550 Bluetooth stopped working (Unknown USB device - device descriptor request failed)

So in desperation I just went into BIOS settings to see if there are any BT settings that may have become disabled. I didn't see any, and I didn't change any other settings.
After rebooting, BT and my mouse is now working again.
Something seems to be really flaky here, but hopefully this will help others with the same issue.

Hello, ok so after recently doing a clean install of windows 10(please don't get me started on that mess). my system has been plagued by random usb drops. it's where everything connected via USB suddenly disconnects then reconnects out of the blue. well I was using a Rosewill Transenclosure RX200R-APU-25B USB 3.0 dual 2.5 hard drive enclosure and transferring files from one drive to another (connected via USB) when one of these drops occurred. Now whenever I plug in my hard drive enclosure in a windows warning pops up and says "the USB drive that was just connected Malfunctioned and could not be installed" and in system manager it says "Unknown USB device (device descriptor request failed)". I know it's not the hard drives because I can remove them and use another connection and they work fine, all files intact. It's just now my hard drive enclosure won't recognize. This also happened to a "Loot crate exclusive Game of Thrones stark sigil 4GB USB stick I plugged in and turned into a windows 10 install drive. When I plugged it into use it after the windows media creator said it was done the USB stick was raw and unformatable. Please help. Microsoft keeps telling me it's a hardware issue even though it happened on two different systems. both operating windows 10 never any issue with windows 7 or 8.1

Now whenever I plug in my hard drive enclosure in a windows warning pops up and says "the USB drive that was just connected Malfunctioned and could not be installed" and in system manager it says "Unknown USB device (device descriptor request failed)". I know it's not the hard drives because I can remove them and use another connection and they work fine, all files intact. It's just now my hard drive enclosure won't recognize. This also happened to a "Loot crate exclusive Game of Thrones stark sigil 4GB USB stick I plugged in and turned into a windows 10 install drive. When I plugged it into use it after the windows media creator said it was done the USB stick was raw and unformatable. Please help. Microsoft keeps telling me it's a hardware issue even though it happened on two different systems. both operating windows 10 never any issue with windows 7 or 8.1

As per the query we understood that you are facing issue  with error in USB device in your Lenovo G710 laptop.
 

Unplug all USB peripherals connected on the machine
Run Windows Update to get the latest patch and bug fixes (reboot if necessary) and try the USB devices.

If still the issue persists then please follow the below steps:-

Go to start button right click on my computer.
Select properties in left upper corner you have 4 options select ?advance system settings? option.

3.System properties pop up comes under that select hardware option and select device installation settings and disable the auto driver download option and select save changes and restart the system.
 

Now try to update the driver and check for the issue.

We have some symbol windows mobile-based handheld devices that we use in our business.  We connect these devices to our computers to copy files to.  This all worked fine until recently the devices come up as failed.
It appears in device manager with a yellow exclamation point.
Windows cannot start this hardware device because its configuration information (in the registry) is incomplete or damaged.  (Code 19)
I've tried all the solutions I can find on the web and none of them work.
Including:
Uninstall the device in device manager and scan for changes.  It downloads the driver from windows-update and returns to code 19.
Tried hardware troubleshooter which basically seems to automate the process of uninstalling and scanning.
I've tried removing windows media player 12 feature and adding it back.
I've tried deleting USB controllers from Device Manager and rebooting, it re-detects them all and still gets code 19 on the device.
I've tried deleting upper and lower filter values in the registry which I found out later is mainly for CD/DVD devices, that didn't fix anything.
I've tried removing and re-installing Windows Mobile Device Center and driver pack and that did not fix it either.
Additionally Canon Powershot camera gives us similar results.  MTP USB Device failed.  
This is so far happening on 3 of our workstations that we know of.

Do you have Kaspersky installed? Kaspersky Filter driver caused that any USB device could not connect.

 

It is suggested to have a look at the following registry. Make a backup of it and remove the Upperfilters setting:

[HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\Class\{EEC5AD98-8080-425F-922A-DABF3DE3F69A}

“Upperfilters” = klfltdrv

 

If the issue persists, please find a working machine and compare the registry settings.

Hello,

Last night i was trying to update my AMD SATA AHCI Controller driver, but i shouldn't do while being sleepy....

My hand moved before my brain realised...I uninstalled the device BUT i also checked the option for deleting the drivers.

Now, i get the error while booting :

What i realise is that windows are trying to find the device needed to boot ( stored on boot info) , but device is not there.

Ι already searched the forum for similar problems and already tried to do a Startup Repair from Windows Installation Disc repair menu (3 consequent times as written), with no result.

No restore points either.

Is there any way to manually fix this or my only path is a repair install ? All my data are safely backed-up.

Well, i found the solution after some 50-60 Google results reading...

Solution was found in this page.

Analytically :

I loaded Recovery Console from my Windows 7 Installation Disk.

Had the AMD SATA Driver folder copied to a cd from my second pc. Driver directory was "W764A"

Ran diskpart and list volume to check the cd's volume letter

Ran the following command ( D: is CD ROM, D:\W764A is Driver Directory, C: is Windows Installation ) :

I typed exit to exit Recovery Console, rebooted and voila....Windows Started normally...

Can any help with this problem? Have a Pavilion G6 with no USB 3.0 as I am aware. USB (3 ports) not working.However, when the plugged the mobile, the mobile does get charged. Device Manager shows "Unknown USB Device (Device Descriptor Request Failed)".   Running Windows 10 64 bit. Tried everything like uninstalling, updating etc as the information on the internet. After installing Windows 10 anniversary update, I get the following error in the device manger: Unknown USB Device (Device Descriptor Request Failed)

Tried uninstalling and restarting PC. Reinstalled Windows made from media tool. Nothing fixed it. Did a google search and nothing has worked. No USB drivers on the PC maker's website.

Any ideas?

You can try going into device manager > select show hidden devices and delete all "inactive" usb device appearances. In my example below, there would be four. Once done, reboot and try your device again:

Fresh install of Windows 10 and the last driver I need to sort out is the above.

I believe it is my powered USB 3 hub to which I plug in my keyboard and mouse. Technically it is working but I don't like seeing yellow triangles on devices.

So are there any fixes?

If it's a generic USB 2.0 Powered Hub then technically there's should be no drivers needed by Windows, if it's a USB 3.0 Hub then I'm unsure (since I don't own any USB 3.0 Hubs).

Might be a Windows bug, basically Windows can't find the description information to display the Hub's name in the Device Manager example: Generic USB Hub, Root Hub Device, etc....

I'm pretty sure everything is working as should, you're good minus the minor annoyance if all device connected through the USB Hub is recognized by Windows; if you right click Properties on the yellow triangle it probably says "The Device is working properly".

Im running windows 10 on an HP laptop, 64 bit, and i just got a micro usb to usb device so i can transfer stuff onto my phone. It worked when i first got it but now it comes up with a Device Descriptor Request Failed and a Code 43 error. Ive tried updating
the drivers, restarting my laptop, and plugging it into different usb ports but it still comes up with the error. Any suggestions?

I know that a similar question has been asked before but, not with the last part of the message, (Device Descriptor Request Failed). I get this, in Device Manager, and I have searched for an answer on the net. Also it is a Code 43. I went through my laptop, with an HP Technician this morning, I might add at a cost of 5p/minute, for 30 odd minutes. He has said that because I had gone over to Windows 8.1 and the recovery discs I made will not load, to put back in Windows 8. I have to send the laptop back for re-installing the OS, because Windows 8.1 do not have the update for my problem. Also the R.H. USB socket has a failure ID:0GAPTK-6T7716-MFPWWK-8D1Q03. I personally think that this is all what the problem is. a duff USB. Is this a cop-out???
My question is; has anyone heard of Windows 8.1 having driver WHQL problems?? Very.

Any/all ideas, appreciated.

ps, I only found out the problem with the usb when I tried to download a Mobi file to a Kindle for my wife. The other usb's work OK.

Is there only one USB port on the right hand side? Sometimes manufacturers connect internal devices via an unused USB port internally. USB ports are usually in groups of two, seeing only one port is usually a sign that the wiring has been redirected internally to something else like a Bluetooth adapter or wifi adapter. It may even be a card reader or touchpad. Is anything like that not working? If it is start looking for a driver for it.

When my girlfriend connects external USB device to any of the USB ports on Windows 10 Home she gets message:
USB Device Not Recognized.
She can not charge USB device via USB port.
I went to device manager and under USB Controllers I see following:
Unknown USB Device ( Device Descriptor Request Failed).
I tried some tricks I googled for this message but none of them returned result.
She didn't get any of the drivers with this device which needs to charge.
What should I do to be able to charge this device via USB?

Answer:USB Device Not Recognized Device Descriptor Request Failed

Try the device on a different USB port. This will figure out if problem is caused by a broken port.
Connect the device to another PC. This will figure out if the external device has problem.

Open Device Manager and expand category Universal Serial Bus Controllers and locate the USB device which is not being recognized by Windows.

Right-click on the device and select Uninstall from the context menu.

After finishing the uninstallation, restart your PC then Windows will reinstall the device automatically.

The problem can be caused by faulty USB driver. In this case, updating driver would fix the problem.

Obtaining Hardware ID's to help Identify Hardware
- Go to Start and type in "devmgmt.msc" (without the quotes) and press Enter
- Expand the "Unknown USB Device" category by clicking on the + sign to the left of it
- Right click on the "Unknown device" (if there's more than one, do this for all of them) and select Properties.
- Then select the Details tab.
- In the dropdown box, select "Hardware IDs" - and match the info to one of the following descriptions:
- - If the Hardware IDs description contains VEN_ and DEV_, then let us know the 4 characters immediately after the VEN_ and the 4 characters immediately after the DEV_

Greetings folks

I have a problem that bugs me for years, I never intentionally solved it, sometimes those errors disappears and all USB ports work fine, sometimes they do not. So anyone can help me please, it would be great.

Some of my ports do not work all the time, I cannot identify a specific moment. Inside the Device Manager, Universal Serial Bus controllers, I get 1, 2 or more "Unknown USB Device ( Device Descriptior Request Failed )", sometimes more sometimes less.

CPU: Intel Core i7 3770
Motherboard: AsROCK H77 Pro4/MVP
Windows 10 all available updates downloaded and installed as of today, and everyday. The problem appeared on Win 8 or 7, not sure when. So it's not necessarily Windows 10 specific. I ported it to 10.

Bios is up to date. Chipset drivers updated, various solution tried from googling, I even managed to make all USB ports stop working testing some of these solutions, I had to restore windows from a ghost image I had.

It's like these USB ports remain suspended sometimes.

Any advice on what to try? The most annoying part is that from the frontal USB ports some stop working randomly too.

EDIT: The problem is the motherboard. The computer was checked at a computer service, and it seems the time has come for it to be buried.

My computer keeps telling me Unknown USB Device (Device Descriptor Request Failed), so I looked up how to fix it and they told me to uninstall it then scan for hardware changes and it will reappear working properly. Only it did not come back working properly, it came back the same. I am also 98% sure that this "unknown USB device" is my built-in webcam that has never once worked. So if someone wants to help me out and allow me to use my webcam that would be great!

The issue that your having is most likely the one discussed in this article.
 
If you have tried the USB 3.0 (x2) and USB 2.0 (x1) ports with the same issue, consider the following:
1. Unplug all USB peripherals connected on the machine
2. Run Windows Update to get the latest patch and bug fixes (reboot if necessary) and try the USB devices.
 
If the problem remains, consider going back to Windows 8 by doing a Refresh or Reset to isolate any issues on the OS level.

Hi, i am having weird problem on windows 10. i fresh installed today and was installing drivers and other softwares. i couldnt set fingerprint because it says biometric device is not enabled. then i realized this error code : 43 . i dont know if it s related with fingerprint or not. also i installed old driver of integrated webcam in compatible mode win 7. i dont know if it caused this problem either. any help please?

Done!it was Fingerprint. fingerprint socket was unplugged. after i re-plugged the socket windows recognized biometric device. then i installed thinkpad fingerprint driver 6.0, all worked fine. there is no unknown usb device anymore. regards,

1.)So my mouse(Corsair Harpoon RGB) was recently just working fine until one night i unplugged it and put it into computer and on that computer it didnt work so i went back to my laptop to plug it in there, but it showed the same problem it didnt respond the logo didn't light up so i went to the device manager and it said *title. So how do i fix this is there something with my mouse is there something with my software?
Oh yea and on top of that i have 2 more issues that i jut can't solve by myself.

3.)DVD Drive(optical drive) doesnt show up on the device manager but it sounds like it's working and it opens and closes.
4.)Front ports of my PC doesnt work if i plug an USB in it doesnt do anything (it does work perfectly in the back)

I don't know though if i should make more threads but they somewhat relate to each other,

I am running the Lenovo Ideapad y510p (the GT755m SLI version). I had my USB mouse and my flash drive connected to the 2x 3.0 USB ports (the ones on the left), everything was fine until suddenly both of them stopped working. When I try connecting the flash drive nothing happens, however, when I try connecting the USB mouse it says Unknown USB device (Device Descriptor Request Failed) in the device manager, windows error message "USB Device not recognized" also pops up in the bottom right corner. When I try connecting either of them (the mouse of the flash drive) to the 3rd USB port (the usb 2.0 port) they both work perfectly fine without any errors at all. I would like to know what may have caused that error and if there is any solution at all. Thanks in advance, Mersif.

Hi,

Since the 11/18/2012 event viewer (Event ID 219, Task Category 212) has been posting a warning:

The driver \Driver\WUDFRd failed to load for the device WpdBusEnumRoot\UMB\2&37c186b&0&STORAGE#VOLUME#_??_USBSTOR#DISK&VEN (followed by name of USB device)

It only occurs on start up if I have a USB memory device plugged into a USB port. The USB memory device functions perfectly and the error never occurs when I plug in and out a memory device after boot up.

The error only occurs for memory devices like memory sticks, external USB hard drive does not cause this error.

Checking back to around the 11/18/2012 I see nothing that I have changed/installed that might indicate something that has caused this. No other Windows 7 failures or issues.

Booting a second hard drive with windows 7 on the same system (I have this second installation for testing faults etc), the issue does NOT occur.

From checking forums and searching I have found a few others with this issue and a few articles on its cause

WUDFRd failed to load for the device

Event ID 219 is logged after you install Windows 7 or Windows Server 2008 R2

I was just curious as to why it would suddenly start occurring after a particular date when as far as I can tell I have not made any changes to anything around this date.

Also I wondered if there was a fix to stop this, other than removing a memory stick before boot up!

Any thoughts?

I just started getting this exact same error message logged in Event Viewer when I plug a USB stick into a USB port. It happens with two different sticks from two different manufactures, but it does not happen every time I plug them in. It seems that it may mostly happen the first time I plug a stick in after I have booted up my PC.

Have you been able to find a solution to get these error codes to stop logging?

after looking at the minidump i thought it was due to a usb vidio/audio capture device so i tried updating the drivers and windows update couldnt find any and i cant find any online.

my niece has a packard bell 266 with 160mb and is running win me.

when she turns on the comp she gets the post test and then it goes into options and guides her to safe mode.
after safe mode her desktop screensaver comes up and then new hard ware found but then nothing.
we managed to get scan disk to run once but nothing else, it just hangs on the screensavers.

the message after trying to run new hardware is:

Mods, if you feel that this thread is more of a "Windows" issue, please feel free to move this thread.

Here last year I did my second build because I was having "boot" issues. Wasn't certain what exactly the problem was at the time. But something some how screwed up the boot sequence in my system, and where everything was pretty much "outdated." I just went ahead and did another build.

Though, since then I've been pondering around with the thought of using a SATA to IDE adapter so that I can retrieve what ever salvageable data I can from one of the two HDD's that was in my last build. So I finally broke down and ordered a couple different one's earlier this week.

Unfortunately, I've been running in to some problem(s) when I try to launch with the IDE in place.

Upon initial launching, the boot sequence seem to take some time longer than it normally does with out the IDE HDD in place. Though after a 15-20sec lapse I'll get an error screen displaying an windows error code of "0xc000000e" with a prompt message below stating "The boot selection failed because a required device is inaccessible"

I searched all over online to see what that error code represents, only to get "pages after pages" of sites with each site telling me something different.

I have no intentions of using this as a third HDD in my system. I just want to retrieve select data from it.

hello, i'm trying to open the game driv3r and i get the message "failed to create directX. Not supported display adapter present..." my card is Mobile intel(R) 945 express chipset family(driver updated) and the laptop has 1gb ram with the cpu at 1,80mhz..

Try this:

go to Start / Run / type dxdiag and press enter

on Direct X Diagnostic:

click on every tab (screen / sound / music / network / etc)... and run the tests. See which one is giving out an error, and come back to investigate further on that.

Could also be the program you are trying to run is not compatible with the "latest" version and is actually trying to use an older version.
Otherwise try reinstalling directx and then the program.

I am running Windows XP SP3 and after reinstalling my system after a recent failure I receive the message "Failed to Create OCX Control" every time I log on. The system seems to operate properly, but the error message has piqued my curiosity.

Can anyone please help me identify what is going on and ideally how to correct the error?

is the full error message by any chance: "Create Overlay failed. Please lower your screen resolution or color depth and try again."

If so are you plugging a laptop or desktop into a secondary display as an extended desktop?

First try lowering the display settings to a lower resolution, especially the secondary display or TV.

or goto Display Properties & click the settings tab > and untick/check the box by Extend my Windows desktop onto this monitor > click OK. ( however this will only allow you to view the DVD on the primary monitor )

Senario:
user is using "Wordpad";
Click INSERT, then OBJECT;
receive's the basic OLE interface;
click "CREATE FROM FILE", browse to the file, click OPEN;

Instead of the picture being dropped into Wordpad, we get the error "Failed to create OLE object".

SOmetimes there is a frame where the picture is supposed to be, but no picture... only the file name inside the box.

Searches on the web don't reveal much, and most reference Windows NT 4.0... We're in XP (home/pro/ent)

We've checked the file associations, and the registry entry for HKEY_CLASSES_ROOT\.jpg... nothing seems to resolve it.

Any ideas??? Anyone??? Your help would be greatly appreciated!!

When I used printmaster16 today I wanted to use some pictures that I have there. They were ones that I imported from another site. When I click on them to select it says "Failed to create picture". I've used them before but I know it has been awhile since. Could someone help me out and tell me how I can use them again? (if possible) THANKS

Have a new Inspiron 13 5000. It has completed all of its updates.  I have tried several times with a couple of different USB thumb drives and each time the result is the same message that something has gone wrong -FAILED.  The USB drives are 32 GB

Answer:Create USB Recovery - Failed

Have you made any changes to the OEM drive configuration?
Have you run a system File Check?  Admin command prompt sfc /scannow
You might try cleaning or removing any current partitions on the flash drive.  Do you know if it has any proprietary software on it?
Are  you checking the box to copy over system files?

I'm experiencing slim keyboard not being recognized in the Windows 8.1
The keyboard was working fine until just one day it just stopped working.
I tried factory reset literally 8 times along with most recent BIOS update and dell's recommended update software. + power setting --> disabling power saving on USB
It still come up as unrecognized each time even with fresh reinstall.
I was wondering if this is a hardware issue or just software bug from factory reset.
I don't think there is a available driver for this kind of problem is there?

it's because you don't have enough space in the hidden system reserve partition, first can you make us a print screen of your disk management...right click on computer icon > management > disk management > right click on the reserve partion and show us property like in my image

The creation of the recovery medium was not terminated properly because of a media error. A repetition allows the program (see Solution Center) not to do. It seems that the flag does not takes place only after successful completion. A corresponding in a ini or a registry - entry I could not find. What can I do to still create a DVD?

If the ThinkPad is still under warranty, call Lenovo service and explain the situation. They will send you DVDs at no charge. At least that has been my experience in North America.
